I have noticed increasingly lately that my concentration, memory, both short and long, ability to think and concentrate and functioning is notably impaired. I seem to be in one place but another in mind, never really with it and often finding myself not really listening to people even going into a trance and finding it hard to snap out of going into myself. I have been under a lot of mental stress lately, loss of someone close, generalised anxiety, became a new mum, moving house i could go on. Could this just be severe stress or could i be going mad?
Yes, absolutely severe stress can cause many issues. This certainly includes your mental functioning. Difficulty concentrating, lack of focus, grief issues ("lost someone close"), new mother, moving. Your stress is very high at present.
Also generalized anxiety can be a separate issue or it could be connected with the stress you are experiencing. I do hope that you will consult your Dr or other health care provider to discuss these issues in depth. If you do have depression it is very amenable to treatment with the caution that treatment is more helpful the earlier you take yourself to your Dr. (many of the symptoms you mention are indicative of a depressive disorder). Sounds like stress to me. You have a lot to worry about and a lot on your plate. Give yourself some time to adjust to the new situations in your life.

You’re not going mad. When someone experiences a great deal of severe anxiety, its normal to kinda zone out. All that stress tends to drain you to the point where you just have nothing left in your tank. What you gotta do is find an outlet, something you can do to take your mind off things. Trust me, you’re not going mad. Good luck

Definately severe stress. Ive been through the same in the past. Had tests, brain scans etc. If you are worried go to your GP. I thought I was going mad but it was just stress overload. Try and take some time out and relax even if its the odd half hour if like me you are running here there and everywhere.
If youve got time an exercise class is good for stress. Just a thought, have you got a young baby? I had bad post natal depression and it made me completely phased out!!! Get it checked out so you can put your mind at rest.

The first thing that you would want to consider since you are a new mom is Post Partum Depression this also includes severe stress and anxiety. Anxiety can definitely affect your concentration and cause the symptoms that you describe. Severe anxiety can also affect your physical health. It would probably be a good idea to talk to your doctor about this problem. If it is disrupting your life there are plenty of safe medications that can help you. You have a lot going on in your life right now along with the grief of losing someone close. I hope that you will talk to your doctor because there is help. The feelings that you are having can also affect your new baby as they sense that something is wrong and it can effect them in a negative way. Please take care of yourself. Honestly there is no such thing as going mad. You just need some professional help.

You hit the nail right on the head when you said "I seem to be in one place but another in mind".
It is perfectly common and normal to experience this kind of thing when you are over stressed. Your mind cannot be in too many places at once so it starts to protect itself (and you) by shutting down. Imagine if you tried to save too much information to a hard drive once it reached it’s maximum capacity. Same thing for humans.
You need to start doing some radical "self care". What is the easiest thing for you do to that will give you a "time out" and some quality rest. What is something you can do for yourself each day that would give you time to relax and unwind? A lot of the suggestions in other answers here might be helpful too, but you need to decide what will work for you and then start taking the steps to change the way you are doing life right now.

yes stress can most certainly cause these symptoms, in fact it is often one of the earliest signs that you are under stress.
Stress can affect you in som many ways, psychologically, behaviorally, physically etc. it really does need to be managed before the symptoms develop further.
